Pamphlet Order Form

The GI Society provides free patient information pamphlets to the public as part of our charitable mission.

Before you place your order please see the following notes:

• Maximum order per pamphlet is 200 copies. Contact us for special requests.
• Please specify the number of individual pamphlets you wish to order (1-200). Do not order in bundles.
• For bulk orders, please order in increments of 25 (e.g., 25, 50, 75, 100, etc.)
• Only order what you will use in the next 3-6 months
• Bulk pamphlet orders are usually shipped in 2-4 weeks
• We only ship to Canadian addresses
